HIV Medical Case Management
Confidential HIV Medical Case Management services are available to persons living with HIV/AIDS who are working toward leading an empowered life. HIV Medical Case Management services help link HIV positive clients to a continuum of health and social services. These services ensure that clients with complex needs recevie timely, coordinated services, assisting clients to function independently and productively as long as possible. HIV medical Case Management is client centered, focused on empowering the client, goal oriented, flexible, culturally sensitive, efficient, cost-effective, easily accessible and calls for collaboration amongst the community of HIV providers in our service region. The development of a Care Plan will allow you to set and achieve tangible goals. Our trained staff provides coordination of medical, dental, legal, mental health, spiritual, social, financial, and home health care services. Other Case Management services include crisis intervention, advocacy, assistance with governmental programs, and referrals.
Financial Assistance
In some cases, we can provide emergency financial relief to help with rent, utilities, medication, nutrition, transportation to medical appointments, and other essential needs. Your eligibility for these services will be determined according to our agency guidelines.
